Understanding Blood Bank Antibody Identification
Admin July 12, 2026 3 min read
Antibody identification is one of the most challenging skills in blood banking. This guide breaks down the process step by step.
Anti-D (Rh system) is the most common cause of HDFN. Anti-E and Anti-c are also common in the Rh system. Anti-K (Kell) is the third most common cause of HDFN. Anti-Jka and Anti-Jkb (Kidd) are notorious for delayed hemolytic reactions.
Use the rule of three: three or more cells positive with the same antigen pattern. Rule out antibodies by finding at least two cells negative for the suspected antigen that are also negative in the reaction phase.
Enzymes (ficin, papain) enhance Rh and Kidd antibodies while destroying Duffy and MNS antigens. This helps differentiate antibody specificities.
